Arch-last technique for total arch replacement using two-pump system in Stanford type A aortic dissection

چکیده
Methods Between June 2005 and December 2012, 22 patients (16 men and 7 women), aged 28 to 76 years(mean 56.7 ± 13.8), underwent total aortic arch replacement using antegrade selective cerebral perfusion through median sternotomy. Present pathologies were Stanford type A dissection (acute state in 19, chronic state in 3 patients), previous Bentall’s procedure in 3 and previous descending thoracic aortic repair in two patients. Operations were performed with antegrade cerebral perfusion through two options: with right axillary artery, left common carotid artery and left subclavian artery, or without the latter. After anastomoses of descending aorta and proximal ascending aorta operation were performed, the three neck vessels were each anastomosed during heart-beating with two pumps which were used for both cerebral and distal perfusion.

Should the “ elephant trunk ” be skeletonized ? Total arch replacement combined with stented elephant trunk implantation for Stanford type A aortic dissection
文章来源:Acquired Cardiovascular Objectives: To eliminate the residual false lumen in the descending thoracic aorta and improve long-term outcomes of surgical intervention for Stanford type A aortic dissection, we performed the skeletonized “elephant trunk” procedure in the ascending aorta and aortic arch replacement combined with transaortic stented graft implantation into the descending aorta for...
متن کاملMidterm Results of Aortic Arch Replacement in a Stanford Type A Aortic Dissection With an Intimal Tear in the Aortic Arch
BACKGROUND AND OBJECTIVES Stanford type A aortic dissection is a potentially catastrophic event that requires surgical repair, on an emergency basis. The extent of arch repair that should be carried out during emergency surgery of this type is controversial. This study was designed to evaluate the results of arch replacement carried out during acute type A dissection. SUBJECTS AND METHODS 28 ...
متن کاملRetrograde Ascending Aortic Dissection after Stent Grafting for Stanford Type B Aortic Dissection with Severe Limb Ischemia
We report a rare case of retrograde Stanford type A aortic dissection after endovascular repair for complicated Stanford type B aortic dissection. A 45-year-old man presented with a sudden onset of back pain and was transferred to our hospital. Computed tomography demonstrated acute Stanford type B aortic dissection with lower limb ischemia. Emergency endovascular surgery was planned for repair...
متن کاملSelective cerebral perfusion with 4-branch graft total aortic arch replacement: Outcomes in 12 patients
BACKGROUND Aortic arch reconstruction is associated with high neurological morbidity. Our purpose is to describe our experience using a 4-branched graft and selective antegrade brain perfusion (SABP) for total aortic arch replacement (TAR). METHODS We retrospectively reviewed the medical records of 12 patients who received TAR, with or without ascending aorta replacement, with a 4-branched gr...
متن کاملSun's procedure for chronic type A aortic dissection: total arch replacement using a tetrafurcate graft with stented elephant trunk implantation.
The Sun’s procedure is a surgical technique that integrates total arch replacement using a tetrafurcated graft with implantation of a special stented graft in the descending aorta, as a treatment option for extensive dissections or aneurysms involving the ascending aorta, aortic arch and descending aorta (1-3). To illustrate our technique for performing the Sun’s procedure (1,2), we present a v...
